DIRECTIONS:
STEP 1 - Download the PDF form below (right click the link and choose 'save file as'; or for Macs, ctrl-click the link and choose 'download linked file').
STEP 2 - Open the form in Adobe Acrobat Reader; fill out the form & make sure to save.
STEP 3 - Lastly, please email your completed PDF file as an attachemnt to: reception@skinmedicalspa.com. (please do not print and fill out by hand).
- New clients should arrive 15 minutes before their scheduled consultation to complete a client profile.
- When calling, please have your credit card number available, so we may hold your scheduled appointment time.
SKIN Medical Spa accepts Amex, MC, Visa and Discover. We do not accept personal checks.
- 24-hour advance notice is required to reschedule or cancel an appointment.
- $100 fee applies to a missed appointment.
- At the request of our guests, please turn off cell phones and refrain from use while in the office.
- Unopened products may be exchanged within 30 days of purchase for product credit only. Opened products and prescription items are not returnable.
